The notice must include all of the following information:

Send your DMCA Notice to Flipiffy’s Copyright Agent at the address or email below. Upon receiving a valid DMCA Notice, Flipiffy will act expeditiously to remove or disable access to the allegedly infringing material and will notify the party who posted the content. We may also provide your email address to that party so they can respond. If the alleged infringer believes the material was removed by mistake, they may send a counter-notice with a similar set of information and a statement consenting to jurisdiction, as described in the DMCA.
